PATIENT CASE
Purpose of assessment task
Nurses have an active role in medication management, including safe administration practices, knowledge of risks such as medication errors, adverse effects and drug interactions. Patient participation in medication management and patient education are also essential to patient safety.
Assignment case study:
Ms Candice Tong, a 56 year old female, was admitted to Deakin Hospital yesterday, the 14th of July, 2019 with upper abdominal pain, nausea and vomiting. Ms Tong has a past history of hypertension, hyperlipidaemia, and is currently being treated with medication for depression. While she was in the emergency department yesterday morning, Ms Tong vomited fluid with a small amount of blood in it. She had an endoscopy and biopsy of her stomach yesterday, and the surgeon has explained to Ms Tong that she has two peptic ulcers as a result of an infection with the bacteria Helicobacter pylori (H. pylori).
Ms Tong has been commenced on medications to promote healing of her peptic ulcers, and to avoid the recurrence of new ulcers.
It is now the 15th of July at 0730 hours.
Ms Tong had vital signs taken one hour ago:
HR: 74 bpm (regular)
BP: 100/60 mmHg
RR: 16 breaths per minute
Sp O2: 98% on room air
Temp: 36.9 tympanic
Candice Tong has been informed by her surgeon that she may go home this afternoon if she feels well.
QUESTIONS
Q1. Assess Ms Tong’s medication chart and identify and explain two (2) significant risks that may be associated with the use of these medicines. (Approximately 400 words)

Q3a. When you assist Ms Tong to find fresh clothes, you notice that there are bottles of the medications ibuprofen and St John’s wort in her bag. These medications are easily purchased from pharmacies and supermarkets. Ms Tong informs you that she takes one of these medications for headaches, and the other because she feels “flat”. Identify and describe one
(1) significant risk that each of these medications may contribute to when used in combination with Ms Tong’s currently prescribed medications (a total of two risks to be identified). (Approximately 300 words)
Q3b. Identify and explain three (3) key points for education you will provide Ms Tong in relation to the general use of over-the-counter medications prior to Ms Tong’s discharge. (Approximately 400 words)

Submission information:
• This assignment task is to be submitted into the designated assessment dropbox for this unit.
• Due date: Friday 6th September 2019 – assignment to be submitted no later than 11.59pm.
• To ensure appropriate support is available should any technical issues arise, you are strongly advised to submit your assignment into the designated dropbox before 4pm on the due date.
• You are responsible for ensuring that the correct version of your assessment task is properly uploaded into the correct assessment dropbox.
Student instructions for the assessment task:
• Read the patient information and questions carefully
• Familiarise yourself with the online material on CloudDeakin and the material covered in the seminars
• Draw on theoretical concepts of principles of quality use of medicines, legal/ethical requirements and the nurse’s role in patient education
• Access relevant contemporary literature to support your discussion. References should mainly include refereed journal articles.
• The suggested readings in the unit are a good place to start. References to websites [eg. Better Health Channel and/or Wikipedia] are not appropriate for this assessment task.
• Make sure you self-assess your assignment against the marking rubric available before submission. This enables you to make sure all the required areas have been covered.
Presentation requirements:
• Front page: to include student name and number, Assignment title and word count.
• Adhere to word limit requirements (within 10%). The word count does not include headings, references page, reference citations and direct quotes.
• A reference list should be provided on a separate page headed ‘References’ at the end of the assignment.
• Do not include a table of contents
• Headings may be used to identify the question being answered

Structure:
You are not required to present an introduction and conclusion to this case study. The answers to the case study questions should normally be organised in paragraphs of approximately 150 words with each paragraph focused on explanation of one idea. There should be a logical progression of ideas as demonstrated by logically linked arguments/discussion made in each paragraph. Each paragraph should commence with a topic sentence.
Academic writing style
The conventions of written English are expected to be followed to ensure clarity of discussion. This includes correct use of grammar, punctuation and spelling as well as the use of appropriate sentence and paragraph structure. It is also expected that word choice will be formal and professional language will be used.
Paraphrasing and direct quotations
Unless really necessary, most assignments do not require the use of direct quotes. Instead, reexpression of author arguments (paraphrasing) into your own words is required. Paraphrasing of author arguments/statements must be supported by a reference. If a direct quotation is used, you must explain how it adds to the discussion and provide a reference as per APA Style guide 6.
