Consider the following scenarios and answer the questions that follow.
Mrs Camillero has been in the residential aged care home where Caro (an EN) has worked for just over a year.She has recently been suffering bouts of nausea and vomiting, pain in her ribs and has lost weight. Caro is upset after finding out her test results.She has bone cancer and is only expected to live for another two to three months.When Caro goes in to help her get up she decides to ask her about her results. Camillero starts talking about her diagnosis at lightning speed mentioning her fears. Caro sits quietly with her and listens carefully. When she begins to calm down,Caro asks her questions to discover her real fear that she won’t be able to see her first great grandchild due in 4 months. She has no real fear of dying as she is strongly religious and looks forward to seeing her husband again. Caro talks gently to her about hope and positive attitudes and she calms down. Caro holds her hand, gives her a hug and promises to ring her daughter to ask her to come in as Mrs Camillero needs to talk. Whilst Caro assists her to shower and dress her they reminisce about Mrs Camillero’s big family and she even laughs about her youngest brother who has just remarried at 75. Caro has written all this information into Mrs Camillero’s folder and she reports to the care manager, RNs, other care workers and some allied health personnel. Mrs Camillero is not actually afraid of death as she is a strong Catholic but is desperate to see her first great grandchild. She also reports on the desire for advanced directives to be part of the care plan and that having large family numbers visit and pain control remain a worry for her.

Mr. Joe 72 has lung cancer. He is admitted to the hospice today. He was a frequent visitor to the hospice day center over the past few months, and several staff has noted has a positive attitude and seems to have been coping physically and emotionally. Joe’s condition has deteriorated now and he is not expected to live for more than a few more days. He is anorexic, not eating and on IV and clear fluids. He is getting Patient controlled analgesia (PCA) morphine and other nursing measures such as massage, positioning, gentle exercise etc. He is alert but extremely agitated and depressed at the moment. His wife Margaret can’t understand his reaction and doesn’t know how to manage the situation and is finding his agitation distressing.
