In consultation with your WPS, select three (3) clients with a disability. If applicable, you may use the same clients as for Portfolio 1.
In this activity, work with your clients and their support team of carer(s) and family, where you are to contribute, assess and respond to the ongoing skills development using a strengths-based solution strategy to respond to both routine and unpredictable problems relating to the clients’ care relationships.

Task 1: Contribute to the clients’ skills assessment
Using the social profile form in Appendix 1 and Who Am I in Appendix 2, facilitate and access a wide variety of choices that assist three (3) clients with a disability to reach their personal goals. You will need to:
• observe the clients’ skills and competencies in a way that respects the clients’ rights
• engage the family/or carers/ relevant people in the skill assessment
• include carers and family members as part of the support team
• assess and respond to changes in the care relationship
• monitor and promote carer rights, health and wellbeing
• in consultation with your supervisor, record all observation accurately and objectively, using the skills development plan below.
Work and contribute to the clients’ skills development, using a person-centred approach, effective communication, collaboration, their Individual Plans and any other resources necessary.
While conducting your meeting, you will need to ensure:
• your oral communication skills maintain positive and respectful relationships with the person with a disability
• you appropriately use non-verbal communication strategies.
Note: Your assessment must involve real interactions with people with disability, their families/carers/relevant others and the clients’ individualised plan.

Description: For this activity, you need to support at least one client with a disability by working with them to identify skills and interests and sourcing options within the broader community.
Task 1: Support community participation and social inclusion
From the selected three (3) clients, choose one to support by working with them to identify skills and interests and find matching options within the broader community.
During this task, you need to:
• identify skills the client is either already developing or would like to develop in the future, using a strengths-based approach
• identify the client’s current interests and things they would like to engage in, participate in or do in the future, using a strengths-based approach
• assist the client in finding options in the community that suit or match their identified skills and interests.

For this activity, you need access to and use of suitable facilities, equipment and sources as:
• the individualised client plan and any relevant equipment outlined in the plan
• appropriate and local resources, programs, agencies, transport services, aids and equipment

Description
Use the Wellbeing Profile template in Appendix 7 to assess and respond to client’s needs and implementation of the actions that were identified in assessment and review/evaluation of client outcomes.
Each Wellbeing Profile must include details of:
• alcohol and another drug usage, history and behaviours
• medical history
• mental health conditions
• suicide and self-harm risk
• psychosocial resources and support
• assessment of recovery capital
• individual strengths
• recovery goals
• Case Summary Sheet (including treatment plan).

Task 1: Comprehensive Client Assessment
Under the supervision of your WPS undertake a client assessment for three (3) selected clients, using the Wellbeing Profile template in Appendix 7.
During this meeting, you will need to engage with each client in a manner that enables you to:
• promote the principles of ‘recovery’ and ‘recovery-oriented’ practice
• establish the context for a self-directed recovery relationship
• invite the clients to share information
• gain trust and confidence to work collaboratively with the clients.
Task 2: Preparation for a client meeting [to be carried out for each of the three (3) selected clients]
Take some time to prepare for your meeting, review the client’s Wellbeing Profile in Appendix 7 and any other relevant client information available to you in the client’s file.
As part of your preparation, think about the principles of recovery and recovery-oriented practices and how you will promote them during your conversation. The list of actions below will assist in structuring your client meeting as you should ensure your conversation is professional and that you provide your client with the best possible outcome.
It is important that, during your meeting, you demonstrate how you achieve the following outcomes.
a. Promote the principles of recovery and recovery-oriented practice.
b. Establish the context of the meeting to ensure you promote a self-directed recovery relationship.
c. Invite the client to share information.
d. Gain trust and confidence to work collaboratively with the client.
Task 3: Conduct the client meeting

Description
Continuing with your three (3) selected clients, collaboratively develop, implement and review a plan for recovery using a recovery-orientated approach. Throughout this activity, you must show evidence of your ability to complete the following tasks.
a. Share and collect information to collaboratively inform and plan for recovery.
b. Facilitate a collaborative planning process for recovery.
c. Collaboratively implement plans for each of your clients.
d. Develop and maintain effective working relationship with the care support network(s).

Task 1: Share and collect information to collaboratively inform the plan for recovery
As part of developing and providing clients with a plan enabling them to recover and transition, you will need to share and collect information that assists you to work collaboratively with the client and to develop their recovery plan.
In this task, you need to apply the following aspects to each of your three (3) selected clients.
a. Work in a recovery-oriented framework that respects client experience, culture and unique recovery journey, within the previously agreed roles and agreed leisure and health work/client relationship.
b. Work collaboratively with the client to discuss and determine what information and sources of information needs to be collected and accessed.
c. Clearly explain any organisation or program requirements, including the commitment to access and equity, and limit to confidentiality and obtain consent.
d. Work within best practice principles and organisational policies and procedures to conduct any/required formal assessments to gather and document information and to agree sources to explore and clarify the client’s preferences, needs and understanding.
e. Work with the client to identify the range and potential effects of social and other barriers that are impacting on the client.
f. Complete the client’s Individual Program Plan in Appendix 6.

a. Using effective communication strategies based on client choices, preferences, values, needs and goals. Discuss different planning options and tools to collaboratively work with your clients to develop a plan for recovery and transition.
b. Endorse client choices for personal wellness, development of self-efficacy, cultural requirements, values, meanings and purpose in life.
c. Discuss client priorities to achieve goals, including self-advocacy strategies and transition beyond the service.
d. Work with the client to investigate possible barriers or risks and identify other people who can assist in responding to or overcoming these challenges.
e. Develop a Personal Wellness Plan for each client, using the template in Appendix 11.
f. Ensure you and the client practise duty of care and also dignity of risk considerations while promoting independence from the service.
g. Discuss and document your role and the client’s role, including timelines for action.

Now that you have developed wellness plans for your clients, work collaboratively with your clients to implement and document their progress of recovery.
For this task, you will implement the plans you developed for your three clients and use any other information you may need.
Using the Activity Plan template in Appendix 5, work with each of your clients and implement their plan for recovery by actioning the following points.
a. Collaboratively work with your client by discussing their interests and readiness to initiate their recovery plan.
b. As per the agreed times, commence each of the agreed activities/actions.
c. Provide the client with information, resources and education about the opportunities and service options which are relevant to their recovery plan.
d. Provide the client with support in their decision-making, self-advocacy, positive attitude to risk and in gaining confidence and resilience.
e. Maintain regular contact, support and record progress in collaboration with the client.

The care support network is an integral part of the client’s recovery plan. Work with the client to determine who else they would choose to have involved with their recovery process and the roles they will play in the recovery journey.
Following on from your previous tasks, work with the same three (3) clients to action the following points.
a. Identify who will be part of the care support network.
b. Ask the client to provide consent verbally in terms of the type of information that can be shared with specific members of their care network and circumstances in which the information can be released.
c. Build rapport and an effective working relationship with relevant members of the care network and identify the information and support needs of family, carer(s) and friends.
d. Clearly communicate information to members of the care network.
e. Work from a strength-based approach and communicate in a manner that respects the rights, dignity, choices and confidentially of the client with the mental health condition while facilitating the care network to support the client.
f. Provide support, training or services to family, carer(s) and friends based on the identified needs.

b. Be empathic and supportive, with an inspiring and positive approach as challenges occur, treat challenges as part of the recovery journey and part of the learning experience.
c. Provide a quick response to potential incidents or risks and promote safety.

Description:
For this activity, you will need to participate in the planning of a leisure and health program for clients with complex needs. As you have already worked with clients who have complex needs such as mental illness or disabilities, it is strongly recommended that you continue to work with the same clients. From the three clients, you need to work with two clients with mental illness and one client with a disability and develop leisure and health program by:
• participating in profiling specific client needs
• promoting inclusion and empowerment
• contributing to the plan for addressing the impact of client issues, characterises and symptoms
• participating in the development of effective strategies for working with clients with complex needs.

You will complete this activity while working in a suitable facility, using equipment and resources including clients’ Individualised Plans and equipment or resources outlined in the clients’ plans.

Description
Using the information, you have gathered and developed in the clients’ Individual Plans and Activity Plans, select two activities, where you need to provide services to the client(s) in at least two (2) different home or community support settings run by a regulated organisation.

For this activity, you will need access to client Individualised Plans; the use of suitable facilities, equipment and resources; personal protective equipment; legislation and your organisation’s policies and procedures in relation to providing home and community support services.
In this activity, you will need to demonstrate:
• your inter-personal skills
• how you established a positive relationship with the client(s)
• how you clarified tasks
• how you interpreted and followed instructions.
Throughout this activity, you will be asked to:
• determine the requirements of the client individual plan
• establish client relationship in the home
• respectfully operate in the client home
• complete reporting and documentation.
Task 1: Determine requirements of the client Individual Plan
In consultation with your Workplace Supervisor, select two (2) clients. You will provide services in the home of one and you will provide services in a community support setting for the other.
To establish the requirements of your clients’ individual plans, apply the following aspects.
a. Read and interpret the clients’ individual plans, goals, and confirm any equipment, aids and processes you will need to engage in.
b. Contact either the client or carer and confirm the purpose and time of your visit.
c. Prepare for your visit: this may include any equipment, resources, documents and contingencies.

Following on with your previous task, you will need to provide the required service to your client in their home environment.

c. Confirm the client’s consent for the visit.
d. Give the client the opportunity to identify and express any issues or concerns about the visit or other matters.
e. Engage with other members of the home.
f. Check for hazards and other health and safety issues for yourself and your client.
g. Implement any controls to manage risks.

c. Work within duty of care requirements.
d. Show respect and sensitivity towards the client and their home.
e. Work in accordance with legislation and organisation policy and procedures to manage ethical dilemmas, behaviours, and/or possible abuse or neglect.
