16. Read the following case study and document the events that occurred. Be objective and keep to the facts, you are documenting as you would in the clients progress notes following an incident. Use the Focus Charting system outlined in Koutoukidis, Stainton and Hughson, 2013.

You arrived at work at 0645 for a 0700 start on Wednesday 10th July, 2013. You are the Enrolled Nurse in charge this morning on the high care section of Casuarina Aged Care. There is a Registered nurse working the morning shift in the hostel. You are in the office sorting your morning tasks when you hear a crash and someone cry for help. Outside, you find Mrs James an 84 year old resident sprawled on the pavement beside a bench seat; her wheelie walker is on the ground beside her. The Casuarina home’s pet cat sits nearby. Her chin has a deep gash in it which is steadily bleeding. Her left forearm is sitting at a 45% angle and you suspect is fractured. She is alert to time and place but visibly distressed and in pain. You take the scene in at a glance and suspect that Mrs James has tripped up on the home’s cat and hit her chin on the bench seat as she went down. You shout for help and instruct the AIN that arrives to call an ambulance. She does so whilst another nurse collects the blood pressure cuff and the dressing trolley. You apply a pad and bandage to Mrs James Chin to stop the bleeding, and carefully support her arm in a sling. You take a set of observations which are as follows: BP 160/90, Pulse 96, Respirations 18, Temperature 36.5°. The RN from the hostel comes to help and calls the locum GP for the home and gets a phone order for subcutaneous morphine 2.5mg from Doctor Gray. Leaving Mrs James with the AIN, You listen to the phone order and together draw up and administer the pain relief. At 0700 the ambulance arrives and after giving handover to the ambulance officers, they transfer Mrs James to the nearby hospital for treatment. You call Mrs James son who lives interstate, and is her only next of kin. You call Dr Cathcart who is Mrs. James GP and alert him of the fall. You call Rhonda, the Director of Care at the Casuarina home to report the incident. She advised you to complete an incident report and to document the event in Mrs James notes. You then photocopy Mrs James prescribed medications and nursing care plan and fax then to the admitting hospital. Whilst you have been dealing with Mrs James, the RN from the hostel has commenced your medication round for you. Before going back to your tasks you document the incident in Mrs James notes, it is now 0745, you write ‘Falls Incident’ and highlight the heading as is the procedure at Casuarina Aged Care Facility.
